Ive always heard its more of a way to fine tune the air going into the engine moreso than giving HP.  I would start off with something like a cold air intake before a TB spacer....

Biggest bang for the buck is a larger ported TB.

+1

They should give you a little HP (probably wont feel it), but the selling point is throttle response.
Partsdinosaur.    Your engine rebuild needs.

We just put one on a 4.0 jeep with larger Injectors trust me you can feel the difference between the 2 two.  At first was noticable but you can see it runs lean, after the injectors it is a different truck.

On my YJ w/stock gearing and 31" tires, 4.0l FI engine and low miles:

I installed an electric fan, throttle body spacer, cold air kit, and a cat-back system at the same time.  The net result was a 2mpg increase and I could use 5th gear again on the interstate... usually.  But it wasn't a sure fix for 5th by any means.   It felt slightly more responsive, but it might have been all the stickers that came in the boxes.  If you're tearing it apart and have the parts laying around I'd say go for it.  If you're looking to spend a hundred bucks, there's better places to spend it.  For what I spent at that time, I could have probably been about 1/2 way to regearing and actually fixed the problem.
